Online forms for a more personal visit.

You know your pet’s everyday. We want to know it, too. Share their history, complete the form for an upcoming visit, or find a care guide—all in one place, with your Highland team ready to help.

Locally rooted. Relationship-centered. Evansville, Indiana.

Behind every form, a pet and their person.

Life beyond the exam room

Your pet’s health history helps us see the whole picture.

Highland’s virtual check-ins are health risk assessments: a way to bring your pet’s history, everyday habits, and your questions into the conversation. Those details help us build a care plan together.

Set aside a little time.

Allow approximately 10 minutes for a virtual check-in before your appointment, so there is room for the details you want us to understand.

Let the primary caregiver tell the story.

The person who knows your pet’s daily routine is best placed to complete their history and share what has changed.

Start with what you notice.

Changes in appetite, activity, comfort, or behavior—and the questions on your mind—are helpful parts of your pet’s story.

Which form should I complete?

Choose the history or admission form for your upcoming Highland visit. If our team has asked for a specific form, use that one. If you are unsure, call (812) 867-6651 and we will help you find the right starting point.

Who should complete the virtual check-in?

The pet’s primary caregiver should complete the health history whenever possible. Allow approximately 10 minutes before your appointment to share the everyday details, changes, and concerns you want to discuss.

Does completing a form request an appointment?

These forms provide history, account, or admission information. To ask for a visit, use Request an Appointment or call Highland. Our team will help coordinate your pet’s visit.

Where do the online forms open?

The online forms open on Highland’s Jotform pages in a separate tab. The care guides are PDFs you can view, download, or print. If a form or download is giving you trouble, please call our team for help.

What if I have a preparation or recovery question?

Follow the instructions Highland provides for your individual pet. If you are unsure about medication, food, activity, or another part of their plan, call us before making a change. A general guide does not replace that conversation.
